THE REFERRAL PROCESS

COMPLETE THE REFERRAL FORM

If you are a school/education provider looking for Alternative Provision, please email the completed form as attached below back to us, providing as much information as possible. Please send to: admin@hopefields.org.uk


Give us a call on  01642 205233 if you cannot complete the form or require a paper copy - we will be happy to help. 


On obtaining the referral form, we will contact you within two working days to discuss the referral and any further information you may wish to give us. 

LOCAL AUTHORITIES/IF YOUR CHILD HAS AN EHCP

Generally, the Local Authority in which a child resides will consult Hopefields for a place in our school. For further information search 'SEN Local Offer' in your Local Authority webpage. 

COME IN AND VISIT US

Before we agree a place, we will meet the young person and have a brief and informal chat. We like to meet all our new learners individually, in a convenient location, before the commencement of the course.


Communication is key at Hopefields. We keep all parents, carers and school staff up to date at regular intervals with achievement, attainment and general information.


Hopefields believe in significant collaboration between parents/carers, schools, Local Authority teams (and of course ourselves) to create high quality provision. We welcome visits previous to referral from all parties, and would be happy to accommodate and answer questions you may have.


Hopefields love to welcome visitors - come and see what we do and join us on our journey to excellence.

AFTER CARE AND FOLLOW-UP

Hopefields young people have access to our specially trained staff throughout their journey. Leaving us is not an option: Hopefields provide follow up careers advice and guidance - free of charge - for all our learners whilst they wish to access it.


Our main aim is for young people to excel, and to access a mainstream education where possible. A full transition package, staff support and training are available to ensure our young people can return to mainstream provision where and when it is appropriate. 

Please be aware that the form must be completed in full in order to be processed.
